Document header

The masthead + title block that opens every document. Pulled from the document-type catalogue plus the specific register entry being extracted.

Most of these fields are constants per document type (state name, office chain, register name) or per record (entry number, registration date). The renderer ships them as part of the request body; your template just slots them into the masthead.
